Before your preventive sports examination
Pre-examination questionnaire
Every client fills in and signs the athlete questionnaire before a comprehensive athlete examination, ergometry or spiroergometry. It is not needed for spirometry on its own or for the basic anthropometric examination. The form is available in Slovak only. Print it and bring it with you already filled in — it saves time. Copies are also available at the clinic, but filling one in there makes your visit longer.
Under-18s: must come with a parent or legal guardian. The parent usually waits in the waiting room while the child is examined alone, so the parent or guardian fills in and signs the questionnaire in advance.
The results are only meaningful if you arrive rested and prepared. Please follow the instructions below.
Two days before the examination
Avoid any physically demanding training session that takes more than two days to recover from.
The day before the examination
Avoid hard training — an athlete should have a rest day or only a light session. Do not smoke, drink alcohol or eat heavy meals in the 12 hours before the examination.
On the day of the examination
Do not eat in the 2 hours before the examination. Bring your identity card, health insurance card, medical records and the completed, signed athlete questionnaire (in Slovak).

What to bring
- Clean sports shoes
- Sports clothing (women — a sports bra, ideally a swimsuit-style one)
- Toiletries (there is a shower)
- A drink and a light snack
When the examination cannot go ahead
A sports examination cannot be carried out immediately after an illness — leave at least 2 weeks after finishing a course of antibiotics — or immediately after a vaccination.
