Calling is fastest
Call us and we will agree a time during the call. The waiting time for a preventive sports examination is 1 to 2 months, so please call well in advance.
Please do not include health information
Do not write anything about your state of health in this form. We will go through everything necessary by telephone or at the clinic.
Pre-examination questionnaire
Every client fills in and signs the athlete questionnaire before a comprehensive athlete examination, ergometry or spiroergometry. It is not needed for spirometry on its own or for the basic anthropometric examination. The form is available in Slovak only. Print it and bring it with you already filled in — it saves time. Copies are also available at the clinic, but filling one in there makes your visit longer.
Under-18s: must come with a parent or legal guardian. The parent usually waits in the waiting room while the child is examined alone, so the parent or guardian fills in and signs the questionnaire in advance.
